Prevention and Management of Diabetes

What you can expect :

1 on 1 with a primary care provider: Nurse, Nurse Practionner, Physician, Dietician

Testing based on guidelines set by Canadian Diabetes Association

  • Minimum of 4 visits per year for diabetes care
  • Review of laboratory results
  • Measurement of blood pressure
  • Examination of your feet at least once per year
  • Discussion / referral for annual eye exams
  • Assessment of your risk for heart disease and stroke
  • Discussions about exercise, food choices, smoking, mood and sexual function
  • Referral to appropriate care provider

Remember to bring at each visit:

  • blood glucose records
  • blood pressure readings
  • medication list which includes vitamins
